Provide comprehensive in-home physical therapy evaluations and treatment to patients throughout Rockland County. Develop and execute individualized plan of care, document clinical progress, and communicate with referring physicians.
Deliver skilled occupational therapy services in patients' homes to restore functional independence in daily living activities. Conduct home safety assessments, train patients and caregivers, and document care in compliance with payer requirements.
Work under the supervision of a licensed PT to implement established plans of care for home therapy patients. Deliver skilled PT services, document patient progress, and communicate changes in patient status.
Assist licensed OTs in delivering skilled occupational therapy services in patients' homes under appropriate supervision. Implement ADL training, adaptive equipment use, and functional activities per the established plan of care.
